Our staff is committed to creating a warm and friendly environment in order to provide a relaxed and comfortable experience for you. We will take the time to thoroughly understand your concerns and make certain you comprehend all aspects of the treatment plan we formulate specifically for you. We go out of our way to make every person feel welcome-just like they are our extended family.
We ask that all new patients arrive a minimum of 20 minutes prior to their scheduled appointment in order to complete the registration process. Patients who have already completed the registration forms (by accessing them through our web site or by picking them up at our office) need only arrive 10 minutes prior to their scheduled appointment time.
Scheduling and Cancellations: Please call our office to schedule an appointment. If you are unable to make your appointment, please notify us 24 hours in advance.

Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
